We would like to state that the clinic of Prof. Dr. Taner Yiğit (“Clinic”) recognizes your rights as specified in Article 11 of the Law on the Protection of Personal Data no. 6698 (“the Law”). Pursuant to Article 13 of the Law, our Clinic, in its capacity as the data controller, has published the Clarification Text on the Protection of Personal Data on its website at www.drtaneryigit.com, which details the conditions for the processing of personal data, data security, and procedures and principles for destruction.
As the data subject (relevant person), you may send your requests regarding personal data to us using this Data Subject Application Form (“Application Form”) within the scope of Article 13 of the Law and Article 5 of the Communiqué on the Procedures and Principles for Application to the Data Controller.
The rights granted to you by the Law as the data subject are listed below, and you may submit your requests to us in writing and in Turkish using the application method shown in this Application Form.
You may make the following requests with the Application Form:
To learn whether your personal data is processed;
To request information if your personal data has been processed;
To learn the purpose of the processing of your personal data and whether they are used in accordance with their purpose;
To know the third parties to whom your personal data is transferred domestically or abroad;
To request the rectification of incomplete or inaccurate personal data;
To request the erasure or destruction of personal data in the event that the reasons requiring its processing cease to exist, despite being processed in accordance with the Law and relevant regulations;
To request that the operations carried out pursuant to sub-paragraphs (e) and (f) be notified to third parties to whom the personal data has been transferred;
To object to the occurrence of an outcome against the person himself/herself by analyzing the processed data exclusively through automated systems;
To request compensation for damages in the event that the person suffers damage due to the unlawful processing of personal data.
Applications to be made to our Clinic, which bears the title of Data Controller pursuant to Articles 11 and 13 of the Law, shall be made by obtaining a printout of this form from www.drtaneryigit.com and:

Written applications shall be deemed to have been made on the date the document is served on the data controller Clinic and/or its representative; applications made by other methods shall be deemed to have been made on the date the application is received by the data controller.
The response to your application will be sent to the address you have selected within a maximum of 30 (thirty) days from the date the request is served on us.
If you request a written response to your application, your application will be finalized free of charge up to 10 pages. For responses exceeding 10 pages, a fee will be charged per page exceeding the limit, according to the tariff determined by the Personal Data Protection Authority.
We reserve the right to request additional documents to accurately and completely respond to your application and to determine your personal data processed by our Clinic. We are not responsible for any errors or damages that may arise due to the information you provided being inaccurate, incomplete, or not up-to-date.
