In its capacity as data controller, the Clinic of Prof. Dr. Taner Yiğit, primarily guided by the principle of protecting privacy; will record, classify, store, update, and process the personal data of our customers in accordance with the Law on the Protection of Personal Data No. 6698 (“the Law”), within the scope of the Law, under the processing conditions listed in the Law, and will transfer it to third parties in situations and to the extent permitted by the legislation.
This Clarification Text has been prepared in accordance with the current relevant legislation and the rules set out in the regulation, communiqués, decisions, and guidelines published by the Board, particularly Law No. 6698. If there are changes in the Law or other relevant legislation after the publication date of the Clarification Text, and if the Clarification Text becomes inconsistent with these changes, the amended provisions and rules shall apply. All communiqués, decisions, and guidelines published by the Board are monitored by me/us, and the rules stipulated by the Clarification Text are kept up-to-date.
The data subjects (relevant persons) within the scope of this Clarification Text are patients and patient relatives whose personal data are processed.
Pursuant to the Law, your personal data may be collected and processed by the Clinic of Prof. Dr. Taner Yiğit, as the data controller, within the scope described below. The Clinic of Prof. Dr. Taner Yiğit may record, store, update, transfer to third parties, classify, and process your personal data in accordance with the conditions and extent determined under the heading “conditions for personal data processing” in Articles 5 and 6 of the Law. Your personal data specified above is collected via powers of attorney, contracts, printed, and electronic petitions/documents.
The personal data belonging to the data subjects are shared only for the purposes listed above and within the scope of your sharing purpose with our collaborating suppliers and business partners, private insurance companies, laboratories, imaging centers, medical centers, ambulance, medical device, and health service provider institutions, hospitals in Turkey with whom we collaborate for medical diagnosis and treatment, the health institution to which the patient is referred or to which the patient applied, a second physician for the purpose of obtaining a second medical opinion, authorized legal representatives, lawyers we consult, auditors including third parties, your employer, the relevant institution if billing is done to the contracted institution, your companions with the intention of continuing the patient’s care outside the hospital, authorized public institutions and organizations limited to the documents and purposes requested by the relevant public institutions and organizations, and companies providing information technologies and archive services.
Your personal data may be shared abroad within the framework of: the storage of your identity, communication, and health data via various programs whose infrastructure is located abroad; the infrastructure of the system or cloud systems used for storage and archiving activity being located abroad; the maintenance of e-mail contact addresses and our e-mail service infrastructures sent to relevant parties such as private insurance companies in data centers located abroad; the physician from whom you request a second opinion being located abroad; the transmission of the patient’s health data via phone applications with infrastructure located abroad if a second opinion is requested from a physician located in Turkey; and the automatic transmission of the information specified in the contact form on the website to e-mail data centers with infrastructure located abroad.
As personal data subjects, if you submit your requests regarding your rights to the Clinic of Prof. Dr. Taner Yiğit in a verifiable manner by filling out the Data Subject Application Form (based on the Communiqué on the Procedures and Principles of Application to the Data Controller) shared on www.drtaneryigit.com, or with another document containing your mandatory information written in the application form, in writing, or by using a registered electronic mail (KEP) address, secure electronic signature, mobile signature, or the e-mail address previously notified by the relevant person to the Clinic of Prof. Dr. Taner Yiğit and registered in the Clinic of Prof. Dr. Taner Yiğit’s system, to the address below: Harbiye Mah. Milli Reasurans İş Hanı. -2 (D Blok) No:61 İç Kapı No: 6 Şişli/İstanbul, or to the contact address info@drtaneryigit.com, your request will be concluded free of charge as soon as possible and within thirty days at the latest, depending on the nature of the request. However, if the transaction requires an additional cost, the fee specified in the tariff determined by the Personal Data Protection Board will be charged by the Clinic of Prof. Dr. Taner Yiğit. In this context, personal data subjects have the right to:
Learn whether personal data is processed,
Request information regarding the processing if personal data has been processed,
Learn the purpose of the processing of personal data and whether they are used in accordance with their purpose,
Know the third parties to whom personal data are transferred domestically or abroad,
Request the rectification of incomplete or inaccurately processed personal data and request that the transaction made in this context be notified to third parties to whom the personal data has been transferred,
Request the erasure or destruction of personal data in the event that the reasons requiring its processing cease to exist, despite being processed in accordance with the Law and other relevant legal provisions, and request that the transaction made in this context be notified to third parties to whom the personal data has been transferred,
Object to the occurrence of a result against the person himself/herself by analyzing the processed data exclusively through automated systems,
Request compensation for damages in the event that the person suffers damage due to the unlawful processing of personal data.
The Clinic of Prof. Dr. Taner Yiğit assures that in case of a change in the current legislation and any inconsistency between this Clarification Text on the Protection of Personal Data and the said change, the provisions of the legislation shall apply.
